The Zepbound KwikPen is a multi-dose, single-patient-use auto-injector pen. It holds 4 weekly doses of tirzepatide, so you use one pen per month before discarding it. It is available in all six Zepbound dose strengths.

If you take Zepbound (tirzepatide) for weight management or obstructive sleep apnea, the KwikPen may be a more convenient option than single-dose pens or vials. This guide covers how it works, what doses are available, storage, cost, and how it compares to other Zepbound formats.

The KwikPen reduces the number of pens you manage each month. Instead of four separate single-dose pens, you use one KwikPen for the entire month.

Zepbound KwikPen Dose Strengths

The KwikPen is available in all six Zepbound dose strengths. Each pen contains four weekly doses:

Your doctor will typically start you at 2.5 mg for the first 4 weeks, then increase to 5 mg. After that, doses may be increased in 2.5 mg increments every 4 weeks based on your response and tolerance. The maximum dose is 15 mg once weekly.

For obstructive sleep apnea (OSA), the recommended maintenance dose is 10 mg to 15 mg once weekly.

How to Use the Zepbound KwikPen

  1. Attach a pen needle — Use a new pen needle for each injection. Pen needles are not included with the KwikPen.
  2. Prime the pen — Follow the instructions for use to prime before your first dose from each new pen.
  3. Dial your dose — Turn the dose knob all the way until it stops and the dose icon appears in the window. This delivers your full prescribed dose.
  4. Inject — Insert the needle into your skin and push the dose knob. Hold until the dose counter reads zero.
  5. Remove and dispose — Remove the needle and dispose of it in a sharps container. Leave the pen cap off between doses (store with a new needle attached or follow your pharmacist's guidance).
  6. Repeat weekly — Use the same pen for your next 3 weekly doses.
  7. Discard after 4 doses — The pen prevents you from dialing a full dose after 4 weekly injections. Do not try to extract leftover medicine. Discard the pen even if some medication remains.

Important Safety Notes

  • Never share your KwikPen with another person, even if you change the needle. Sharing can transmit blood-borne infections.
  • People with vision problems should not use the KwikPen without help from someone trained to use the device.
  • Do not transfer leftover medicine from the KwikPen into a syringe.

Storage

Unused KwikPens

  • Store in the refrigerator between 36°F and 46°F (2°C to 8°C)
  • Unused pens may be used until the expiration date if kept refrigerated
  • Do not freeze — discard any pen that has been frozen
  • Keep away from direct sunlight and heat

In-Use KwikPens

  • After your first injection, the pen may be stored at room temperature up to 86°F (30°C)
  • Discard the pen 30 days after first use or after 4 weekly doses, whichever comes first
  • Do not put a used pen back in the refrigerator
  • Keep out of reach of children

List Price

Zepbound's list price is approximately $1,060 per month. The KwikPen format may have different pricing than single-dose pens or vials depending on your pharmacy. Check with your pharmacist for the cash price of each format.

Savings Card

Eli Lilly offers the Zepbound Savings Card through Lilly Direct for commercially insured patients:

  • Eligible patients may pay as little as $25 per month
  • The card covers up to approximately $550 per fill
  • Not valid with Medicare, Medicaid, or other government-funded insurance
  • Apply at the official Zepbound website

Insurance Coverage

Insurance coverage for Zepbound varies:

  • Commercial insurance — Some plans cover Zepbound; many require prior authorization. Check your formulary.
  • Medicare — Most Part D plans do not cover weight loss medications. The savings card cannot be used with Medicare.
  • Medicaid — Coverage varies by state.

Common Side Effects

The side effects of tirzepatide are the same regardless of delivery format (KwikPen, single-dose pen, or vial). The most common include:

  • Nausea (about 25% of patients)
  • Diarrhea (about 19%)
  • Constipation (about 17%)
  • Indigestion (about 9%)
  • Abdominal pain (about 9%)
  • Vomiting (about 8%)
  • Injection site reactions (about 6%)

Side effects often increase when your dose goes up. Talk to your doctor if side effects are difficult to manage — they may recommend a slower titration schedule.
