Wegovy HD (7.2 mg): What to Know About the Higher Dose

The FDA approved Wegovy HD (semaglutide 7.2 mg) on March 19, 2026. It's triple the standard Wegovy dose and delivers more weight loss.

What Is Wegovy HD?

It's a once-weekly semaglutide injection at 7.2 mg — three times the standard 2.4 mg maintenance dose. Same drug, higher concentration.

The FDA fast-tracked the review in just 54 days.

Key facts:

Detail Info
Active ingredient Semaglutide

Who Is It For?

Wegovy HD is not a starting dose. It's for people who:

  1. Are already on 2.4 mg Wegovy for at least 4 weeks
  2. Are tolerating it well
  3. Need more weight loss

If you're new to GLP-1, start with our starter guide.

How Much Weight Can You Lose?

The STEP UP trial (1,407 adults, 72 weeks):

Without Diabetes

  • 20.7% average weight loss on Wegovy HD
  • 18% on standard 2.4 mg Wegovy
  • 31% of HD patients lost 25%+ of body weight (vs. 15% on standard)
  • 91% hit at least 5% weight loss

With Type 2 Diabetes

  • 14.1% average weight loss on 7.2 mg
  • A1C dropped 1.7-2.0%

Side Effects at the Higher Dose

Most side effects are the same as standard Wegovy. Two differences:

Altered Skin Sensation

Showed up in 22% of HD patients vs. 6% on 2.4 mg. Most cases improved on their own or after lowering the dose. The FDA is monitoring this closely.

Hair Loss

Higher rates than the standard dose. See our hair loss guide.

GI Side Effects

Nausea, vomiting, constipation — the usual suspects. Only 3.3% of patients dropped out due to GI issues in trials, since most had already adjusted to 2.4 mg.

Serious Risks

Wegovy carries a boxed warning about thyroid C-cell tumors seen in animal studies. Other risks include pancreas inflammation, gallbladder problems, and faster heart rate.

Tell every healthcare provider before any procedure — Wegovy slows stomach emptying and can affect anesthesia.

How to Take It

  • Inject once weekly, same day each week
  • Sites: belly, thigh, or upper arm — rotate weekly
  • With or without food
  • Use the single-use pen — you'll hear two clicks
  • Don't use with any other semaglutide or GLP-1 product
  • Missed a dose? If more than 2 days until next dose, take it. If less than 2 days, skip it.

Cost and Availability

Final pricing hasn't been announced, but it should be similar to standard Wegovy.

  • Manufacturer savings programs should apply
  • Insurance coverage expected to follow standard Wegovy pathways
  • Available at 70,000+ US pharmacies including CVS and Costco
  • Select telehealth providers
  • Medicare: A $50/month GLP-1 demo program launches July 2026
